Bupivacaine 0.125% in epidural block analgesia during childbirth: maternal and foetal plasma concentrations.

چکیده
The maternal plasma concentration of bupivacaine has been determined in 19 patients receiving a continuous epidural block during labour, using bupivacaine 0.125% with adrenaline 1:400,000. The plasma concentration of bupivacaine rarely exceeded 0.18 /*g/ml. Bupivacaine does not accumulate in the maternal blood when such low concentrations are used. The plasma concentration of bupivacaine has also been determined in umbilical venous and arterial blood at birth. The concentration ratio (umbilical venous/maternal venous) at delivery ranged from 0.09 to 0.49 (mean 0.28).

[The efficacy and safety of continuous epidural analgesia versus intradural-epidural analgesia during labor].
OBJECTIVE To determine the efficacy and safety of intradural-epidural analgesia in comparison with continuous epidural analgesia during labor and childbirth. PATIENTS AND METHOD Forty-two women whose labor began spontaneously were enrolled and distributed randomly in two groups. The intradural-epidural analgesia group (IEA, n = 21) received 25 microgram of intradural fentanyl with 2.5 mg of i...

The maternal plasma levels of bupivacaine following epidural administration of 0.5 per cent solution with 1/200,000 adrenaline were determined in twelve patients in labour. After 10 ml (50 mg) peak concentrations occurred 15-30 min later and ranged from 0.22 ^ug/ml to 0.60 ,«g/ml. Plasma levels fell gradually with time after the peak until by 3 hours after injection they were all well below 0.2...

This double-blind prospective study was designed to determine the best dose variables for patient-controlled epidural analgesia (PCEA) and to compare bolus-only PCEA with continuous infusion epidural analgesia (CIEA) during the first stage of labour. Five groups of parturients self-administered 0.125% bupivacaine with 1:400,000 epinephrine and fentanyl 2.5 micrograms.ml-1 using PCA pumps progra...
متن کاملMaternal and neonatal fentanyl and bupivacaine concentrations after epidural infusion during labor.
Labor analgesia using continuous epidural infusions of low-dose bupivacaine and fentanyl may be maintained for many hours. We examined the potential for drug accumulation in both mother and neonate after these long-term infusions. Pregnant women receiving a 10-mL/h continuous infusion of labor analgesia with 0.125% bupivacaine and 2 micrograms/mL of fentanyl were evaluated. Maternal venous and ...
متن کاملEffects on mother and fetus of epidural and combined spinal-epidural techniques for labor analgesia.
OBJECTIVE Epidural (EA) and combined spinal-epidural (CSE) techniques have both been utilized for labor analgesia. This study compared the effects on the mother and newborn of these techniques in labor analgesia and anesthesia. METHODS Forty pregnant women received epidural analgesia with 15 mL of 0.125% ropivacaine (EA group) and 5 microg of sufentanil plus 2.5mg bupivacaine in the subarachn...
